Bloodwork is no longer required in Connecticut. The fee is $30. Please bring identification and come prepared with the name and phone number of the person who will be performing the ceremony. You will need to know your mother's maiden name and the places of birth of both parents. Application may be made at your town of residence or town where the ceremony will be performed. Appointments would be preferred.
